WatchMyHealth offers a tested online electronic medical record system that empowers a doctor to be in control of the management of large numbers of patients.
Any doctor who is interested in using this system for the management of his or her patients may please contact Dr Ramesh Kumar (doctoramesh@gmail.com Mob: 9895372550) or Lt Col (Retd) Anand Nair (asnair@gmail.com).
You may create your Personal Health Record by clicking the 'Create New Account' button on the Home Page and following the instructions. WatchMyHealth does not charge any additional fee for the creation and maintenance of your PHR.
Patients who consult the doctor at WMH Preventive Health Centre in Trivandrum will have accounts created for them automatically.
Use the link Reminders (right menu) to set up reminders (that are emailed to you) for upcoming tests and reviews. If you don't have an email id, you could provide the email id of a relative or friend who could inform you when a reminder is received. For each reminder, enter the Subject and Details (optional), the date of the first reminder, the periodicity (in days) of repeating the reminder and the number of repeats. By setting up multiple reminders with specific periodicities and number of repeats, it is possible to organize your health monitoring for any duration.
The use of this facility need not be restricted to your healthcare needs - you may set reminders for birthdays, social engagements and so on.
You may print out information from your PHR in the following modes, using the right menu options.: 1. Health Summary – a comprehensive PDF report that includes the complete history and progress notes, all laboratory reports and graphic images such as of ECG, X-rays etc. This may be printed out as a hard copy or instantly emailed to your doctor or other recepients. 2. Print Medications List – prints the current list of medications and their dosage. 3. Print Test Report – prints the laboratory report of tests done on any chosen date.
You may enter or edit your lab results using the drop-down options Enter New or Edit provided with each item in the left menu. Before a result is entered, it is essential to know the category to which it belongs. By placing the mouse arrow over the menu items, it is possible to see the tests that each category includes. Click the correct menu item and then click ‘Enter New’. The rest is easy. Enter the date of the test and the values in the appropriate boxes. Make sure the right unit is selected. The commonly used ‘conventional units’ are the default. Once the entry is finished, click on the save button.
You may scan your ECGs and other documents and upload them using the menu opition ‘ECGs, X-rays & Other Tests’.
The Staff at WMH Preventive Health Centre are trained to enter data in your PHRs. A fee is charged in proportion to the volume of data entered.
Click on the link 'Change Password' at the top right corner of all pages and follow the instructions to change your email address and/or your password.

Personal Health Records (creation of account and maintenance) are now a free service. Your PHRs once created will be permanently accessible from our website using your PHR Number (or email id) and password.
Fees for data entry
A data entry fee is charged when our staff (at the WMH Preventive Health Centre) enters data on your behalf proportional to the volume of data entered and the number of images scanned or photographed.
Entry of data generated at the WMH Preventive Health Centre is not charged.
The user may enter data into his or her PHR without incurring any fee.
